Make a Claim

Prior to initiating your claim, ensure you prepare by gathering the following information. If the required information is provided in the correct format, it will help to expedite your claim. Once you have submitted your claim, a member of our Customer Solutions Team will be in contact within 3 business days.


1. Filling out our Warranty Assessment Form 

In the comments field, please include any additional, relevant information. Some information is mandatory and must be completed before proceeding. These fields are marked with an asterisk (*).

2. Photos

Include minimum 3 clear, well-lit pictures for each item - one of the product and one of the specific area or component of concern. For mattress claims, please also include a picture of the mattress label and, where applicable, the mattress base. Photo requirements: .jpg, .png or .pdf format with maximum file size of 3MB.
